ABOUT MY FELLOWSHIP EXPERIENCE
I arrived in Tokyo, Japan on the 2nd of September 2022 and took a flight the next day to Shimane.
My host professor was waiting for me at the airport with a smile and a gift, an umbrella.
This set the tone for my entire visit, welcoming, and accommodating,
I felt extremely lucky every day to have been paired with Professor Masaki Tanito.
He and the whole department of ophthalmology at the Shimane University Hospital allowed me to observe all aspects of their clinical,
academic and social activities; outpatient clinics, theatre sessions, post-op rounds, clinical presentations and wet-lab sessions.
We attended the Japanese Glaucoma Society conference in Yokohoma,
which was a very interesting outing, both educationally and socially.
Prof. Tanito treated some of his residents and me to my very first ever Michelin-starred dining experience.
The language barrier was an issue that created some frustration especially at the conference and throughout the programme,
but the team, especially my host always took the time to patiently explain and translate to me and for me.
Even with that I couldn’t help but imagine how much was lost in translation and how much more
I would have gained and learnt from my time had language not been a barrier.
I had never had the privilege or opportunity to be in a place so well equipped,
where everything and everyone was so disciplined, focused, dedicated, selfless, patient, honest, result-driven, humble, and punctual!
It was a wonderful and eye-opening encounter for me, having to experience patient care and management at its best.
Although I was a bit disappointed to not have had as much practical wet lab exposure to glaucoma surgeries as I had hoped,
I was able to learn phaco cataract surgery at the wet lab and practice the Tanito micro hook ab-interno trabeculotomy (TMH),
a procedure that Prof. Tanito himself pioneered.
I learnt so much about assessment and follow-up care,
I observed procedures and was able to scrub for some surgeries that I had only ever read about.
I had the opportunity to participate in research,
we were able to complete two case reports during the visit and
even presented a paper during the Glaucoma society meeting for Shimane prefecture.
I know and I can with all honesty say that Professor Tanito and his team of exceptional doctors went over and beyond,
including giving me personal tours and all.
They have opened my eyes to so much and have made me altogether a better ophthalmologist and a better person.
I am already working and building on all the knowledge and
skills that I acquired at the same time trying to make changes and teach our residents and students.
Shimane prefecture is one of the most beautiful places I have ever been opportune to visit,
its people an inspiration and I’ll forever be grateful. I thank you for the support and opportunity.
私のフェローシップ体験について
私は2022年9月2日に日本の東京に到着し、翌日の飛行機で島根に向かいました。
空港では、ホストの先生が笑顔で待っていてくださり、傘をプレゼントしてくださいました。
このことが、私の滞在全体の調子を整え、歓迎し、親身になってくれる、谷戸正樹教授とペアになれたことを毎日非常に幸運だと感じていました。
谷戸教授と島根大学医学部附属病院の眼科全体では、外来診療、手術室でのセッション、術後回診、臨床発表、ウェットラボ・セッションなど
、臨床、学術、社会活動のあらゆる側面を見学することができました。
横浜で開催された日本緑内障学会にも参加し、教育的にも社会的にも非常に興味深い旅となりました。
また、谷戸教授が研修医の方々と私に初めてミシュランの星付きレストランでご馳走してくださいました。
言葉の壁は、特に会議中やプログラムを通してフラストレーションを生む問題でしたが、
チーム、特に私のホストは常に時間をかけて辛抱強く私に説明し、通訳してくれました。
もし言葉の壁がなかったら、どれだけ多くのことを学び、得ることができただろうかと思います。
私は、これほど設備の整った場所にいる特権や機会を得たことがありませんでした。
患者ケアとマネージメントの最前線を体験できたことは、私にとって素晴らしく、目を見張るような出会いでした。
緑内障手術の実習が少ないのが少し残念でしたが、ウェットラボで白内障手術を学び、
谷戸教授が自ら開拓したTMH（Tanito micro hook ab-interno trabeculotomy）を実践することが出来ました。
評価とフォローアップについて多くを学び、手術を見学し、本でしか読んだことのないような手術のスクラブもさせていただきました。
研究に参加する機会もあり、滞在中に2つの症例報告を完成させ、島根県の緑内障学会で論文発表もしました。
谷戸教授をはじめとする優秀な医師たちは、個人的な見学も含め、それ以上のことをしてくれたのだと、私は知っているし、正直に言うことができます。
彼らは私に多くのことを教えてくれ、私をより良い眼科医、そしてより良い人間にしてくれました。
私はすでに働いており、習得したすべての知識と技術を基に、同時に変化を起こし、研修医や学生を指導しようとしています。
島根県は、私がこれまで訪れた中で最も美しい場所の一つであり、その人々はインスピレーションを与えてくれますし、私は永遠に感謝し続けるでしょう。
このような機会を与えてくださったことに感謝します。
