個人情報を含む法人文書の紛失について

 

　このたび，本学医学部学務課において，令和４年度における法人文書ファイル管理簿等の更新作業を実施する過程において，個人情報を含む法人文書ファイルの紛失（所在不明）（以下，「紛失」という。）が判明しました。これら法人文書ファイルは外部に持ち出す文書ではないため，誤廃棄の可能性が高いものの，廃棄した事実が確認できないため紛失とし，継続して捜索中です。

　本学では，これまで個人情報保護及び法人文書の適切な管理に取り組んでまいりましたが，このような事態を招き，本学学生をはじめ関係者の皆様に多大なご迷惑とご心配をおかけしますことを深くお詫び申し上げます。

 

１　紛失した法人文書ファイル

　　教育課程編成(大学院)　平成３年度～平成２３年度，平成２５年度　２２冊分（推定）

 

２　紛失した法人文書ファイルに含まれる個人情報

　　当該年度に大学院医学系研究科に在籍していた大学院生の学生番号，学生氏名及び教員氏名　

　　計　６９５人分（推計）

 

３　事実経過

　　令和４年７月２５日（月），医学部学務課において法人文書ファイル管理簿等の更新作業を実施する過程で，法人文書ファイルの現物確認を行ったところ，上記法人文書ファイルを紛失していることが発覚しました。

　　７月２６日（火）以降，紛失した法人文書ファイルが２２冊（推定）になること，保存されている他年度の法人文書ファイルを確認したところ，学生番号，学生氏名及び教員氏名の個人情報が含まれていることを確認しました。

　　当該法人文書ファイルは外部に持ち出す文書ではないため，誤廃棄の可能性が高いものの，廃棄した事実が確認できないため紛失とし，継続して捜索中です。

　　なお，現在のところ，本件に関し，個人情報流出等を含めた被害報告には接しておりません。

 

４　本学の対応

　　当該法人文書ファイルに記載されている大学院生への連絡は，電話連絡及び郵送など可能な限り行います。それでも連絡が取れない方への情報伝達手段として，本件事案をホームページで公表いたします。

 

５　再発防止に向けた今後の取り組み

　　本学では，これまで個人情報保護及び法人文書の適切な管理に取り組んでまいりましたが，このような事態を招いたことを深く反省し，職員等に対して以下の改善方策を徹底させることにより，再発防止に努めてまいります。

 

　【改善方策】

　　１）個人情報保護及び法人文書管理に関する研修を通じた周知徹底

　　２）法人文書ファイル管理簿に記載された全ての法人文書ファイルの目視による点検の実施等適正な管理及び取り扱いの徹底

　　３）法人文書ファイルの管理状況についての実地監査の実施

Re: Missing corporate documents containing personal information

 

At our Academic Affairs Office, in the process of updating the corporate documents file administration system for the academic year 2022, it has been discovered that some corporate documents containing personal information are missing (whereabouts unknown)(“missing” will be used hereafter). As these files are not files that were to be taken outside, while it is highly likely that they were mistakenly disposed of, we couldn’t confirm the fact, and therefore they have been labelled as lost, and we are still looking for them.

Until now, we have been committed to protection of personal information and appropriate administration of corporate documents. We are deeply sorry for the inconvenience caused by this matter to those involved including our students.

 

  1. The missing corporate documents files

  1. The personal information contained in the missing corporate documents files

The student numbers, names and their supervisors’ names of the graduate students enrolled in Graduate School of Medical Research of those years. Total: 695 persons (estimate)

 

  1. Development

On July 25th, 2022 , during the process of updating the corporate documents file administration system, we discovered that the aforementioned files were missing.

From July 26th, 22 files (estimate) were found to be missing, and having checked the contents of the saved corporate documents files of other years, we confirmed that the personal information of the student names and numbers, and their supervisors’ names , was contained in the files.

As these files are not files that were to be taken outside, while it is highly likely that they were mistakenly disposed of, we couldn’t confirm the fact, and therefore they have been labelled as lost, and we are still looking for them.

At this time, regarding this matter, there is no reported damage including the leaking of personal information.

 

  1. Our handling of the situation  

We will try to notify the graduate students whose personal information is deemed to have been contained in the missing files by telephone, post, or the like as much as possible, and as for those who cannot be contacted, we will release a notification of the matter on the homepage.

 

  1. Efforts to prevent recurrence

Until now, we have been committed to the protection of personal information and the appropriate administration of corporate documents. We deeply regret that we have caused this kind of situation. We endeavor to prevent further recurrence of such an incident by incorporating the following policy improvements:

１）Thoroughly spreading personal information protection and corporate file administration through staff training.

２） Ensuring that corporate files are appropriately managed and handled by inspecting visually all the files listed in the corporate documents file administration system.

３） Carrying out on-site audits on the administrative situation of corporate documents files.
