About hospitalization

Hospitalization procedure

Please hand in your consultation card, health insurance card, patient consent form, hospital admission form / warranty card, confirmation of hospitalization form, and questionnaire, at the hospitalization center. 

Also, if you have claimant certificates for latter-stage elderly medical care, welfare-related medical care, and infant medical care, or certificates of ceiling-amount application for high-cost medical care, cost reduction for dietary restrictions, work-related accidents, and social welfare, please submit those too.   

What to prepare 

The hospital has hospital gowns and will lend a gown to the patient for ¥85 per day. However, patients can also bring their own nightwear.

As for leisurewear to wear during rehabilitation, please prepare your own. 

Daily necessities such as toiletries, dining utensils (chopsticks, spoons, cups etc.) change of clothes, toilet paper, and footwear (non-slip, no slippers), can be purchased at the store. (Personal seals are not sold at the store).  

About visiting hours 

Visiting hours are as the following: (changed since: 2011.6.27)

Normal visiting hours: 

6 a.m. – 9:30 p.m.

However, in special circumstances, please go to the nurse’s room in the ward. 

Please try to avoid visiting in large groups or with small children.

Outside of normal hours on Sat., Sun., national holidays, and weekdays, please use the emergency entrance at the west side of the C Ward.

People who come by car, please use the Outpatient Parking.

Service Facilities

Convenience store, service counter, general cafeteria, café, barber, hair salon, post office, ATM, Public Payphone, patient library. 

All beds have a TV (purchase of card and earphone necessary)

Each ward has a washing machine, drier, and refrigerator provided for communal use. 

About hospitalization expenses 

The end-of-month calculated fee will be billed to you around the 10th of the following month. Please pay at the payment counter.
