To first-visit patients

• With your health insurance card and referral form, please arrive between 8:30 a.m. and 11 a.m. If you do not have a referral, you have to pay the specified medical care fee of ¥7700.
• First-visit Reception:
  • The main entrance opens at 7 a.m. The security guard will show you where to go. From 8 a.m., the medical service staff will be available. Please fill in the patient registration form at the waiting area of the first floor, and hand it in along with your health insurance card and referral form at the Reception for New Patients at Counter no.1.
  • In the case of road accident, work-related injury, or occupational accident, please tell the clerk at reception.
  • We will explain the route to the clinical department and give you a consultation card that grants you continued access to all departments. Please understand that on the first visit, it takes time.

About specified medical care coverage

• This hospital is a specifically-approved, advanced treatment hospital. Patients who do not have a referral form have to pay the specified medical care coverage fee of ¥7700.
• Patients who arrive by ambulance or who can claim from public funds for their medical care do not have to pay this fee even without a referral form.
