A Guide to return consultations/visits

• (Please bring your health insurance card and your consultation card) Patients with an appointment slip or patients with an appointment for examination, please check in at the Automated Return Visitor Registration at the waiting area of the first floor, then go directly to the clinical department or the place of examination. Patients who cannot check in at the Automated Return Visitor Registration or who wish to be seen for the first time, please go to Counter no.3.
• Reference tickets are available for patients with an appointment who arrive early in the morning, but priority is given to the appointment time.
•  In the case of forgetting your consultation card, patients must pay ¥110 for reissuance.
