If you have a referral form, please show it at the reception desk.

In accordance with the revision of the Health Insurance Act in April of 2016, when the patient receives treatment at an advanced treatment hospital without a referral form from another medical facility, in principle, the patient is responsible for payment at the initial visit.

First Visit Meical Expenses: for patients who receive treatment without a referral form, the cost is ￥5,500  in addition to normal medical costs.

Retrun Visit Medical Expenses: In the case of continuing to receive treatment of this hospital after being offerd reverse refferal, the cost is ￥2,700 in addition to normal medical cost.

 

Your patient registration card grants you continued access to all clinical departments.

Lost cards will be repurchased.

 

About health insurance card confirmation

We check your health insurance card ones a month. if your card has cahnged, or is in the process of being changed, please let us know at reception.

 

Patients with a patient registration card don't need to write an application form.

 

After the medical examination, take the blue bag containing the outpatient consultation card and other documents toThe payment counter at reception. Please pay at the Automated Payment Machine or at the payment counter.

 

On your next visit, please check in at the Reception for Return Patients or the Automated Recepetion before going to the clinical department ( patients with out an appointment please check in at the Receptino for Return Patients).

 

On yout next visit, please make sure to bring your appointment slip. Patients with appointments for return visits also need to check in at reception.
